What is care and support?
Care and support services, also known as adult social care, are designed to help adults live as well as possible when dealing with illness, disability, or old age. These services can include:
- Help with living at home: Support with daily tasks such as personal care, dressing, and safety at home, provided by a formal home care agency.
- Home care services: Assistance with personal care, such as bathing, dressing, and medication management, through a professional home care agency.
- Meals on wheels: Delivery of hot, nutritious meals to your home if you're unable to prepare food for yourself.
- Help after leaving hospital: Support to help you recover and settle back into your home after being in hospital, including formal home care agency support.
- Equipment and products: Products and modifications to help you stay independent at home for longer, such as bathing chairs, toilet frames and grab rails.
- Telecare services: Devices that can alert call centres, carers or emergency services.
- Support for carers: Help and advice for those who care for a family member or friend, including respite care.
- Supported living and accommodation: Options for living independently with support, including assisted living or shared housing.
- Care homes: options for living within a care home setting.
At some point, we will all need help at home and in the community, whether from family or friends, or from formal care agencies.

How we can help you create a care plan
If you would like adult social care, we can assess your needs free of charge and work with you to create a care plan that is right for you, ensuring your well-being and independence.
We can assist you by setting up care and support, though it is worth knowing that people are expected to financially contribute to some of the options above, depending on their financial situation.

The Advice and Referral Team (ART) is your first point of contact for help and support in accessing adult social care and to arrange care and Care Act assessments and financial assessments.
